Designate a Healthcare Surrogate – make medical decisions.

Every person over the age of 18 should have this document.  This important document allows you to:

  1. Designate a healthcare surrogate to make medical decisions for you if you are unable to do so.
  2. Give certain directions as to certain medical treatments and under certain medical conditions.
  3. Provide HIPAA releases.

Without this document, it can take months and expensive legal fees for your loved ones to be able to make any medical decision for you if something happens to you.
